Diaper Rash On 2 Year Old?

I am crossing my fingers that some one can help me out here!
I have a 2 year old who has had a diaper rash for about 4 months now. It began in January, we just used Pentaten for a bit, but it wasn’t working. So we tried Canesten for a couple of weeks thinking it was a yeast infection (she has had those before and it looked similar) and that did nothing. I took her to the doc February 9th, she thought it looked very yeast-like as well. She put her on a Canesten cream mixed with hydra cortisone, thinking it would help with the inflammation. It seemed to be working within a few days, but began flaring up again. My doc said it could take up to 3 weeks to really work, so we gave her until the beginning of March. When it didn’t clear up, we went to the walk in clinic (unfortunately when we call our doc. office, it’s usually a 3-4 week waiting period). The doctor thought we should try an antibacterial since the anti fungal wasn’t doing anything. We filled that prescrption and began using it right away. Again, like the canesten/cortosone mix, it seemed to start to work. But would flare up again. So a few days ago we took her back to the doctor (our doctor), and she decided it looked like contact dermatitis (my daughter has extremely sensitive skin, so she has it on her arms and legs as well, she also has cradle cap still). We were told to buy all natural diapers, use a barrier cream, and no more baby wipes, use only a wet wash cloth. We have tried that for 4 days now, and she just flared up again really bad yesterday. I am at a total loss, it doesn’t bother her all the time, just occasionally she will complain that it hurts when I wipe, or try to put some cream on it. We’ve tried letting her run around naked thinking that it would help if she “dried it out”, but again it didn’t help at all. It has gotten so bad at some points (this is when it hurts her) that they look like little blisters and have left discoloured marks on her little tush.
Has anyone been through this? Any ideas?
Thanks to everyone who answers!

See if you can get her tested for allergies.
Have you changed anything in the house? Laundry soap, get your couch or carpets steam cleaned, body wash? Is she playing outside more now that the weather is changing? Could be something as simple as the water change. We have run off here and it does a number on the skin since the city has added more chemicals to deal with the run off.
Try adding a handful or two of baking soda to her bath water, letting her dry well afterwards, and a little of her cream.
Also, is she teething? My babies get awful rash during teething.

Take her to a Dermatologist. Hopefully they can help. Call around to find one that has worked with children before.
